Certain events in your personal or professional life may affect your PetSmart benefits.

FlexSmart

Here at PetSmart, we know that when life happens, you have to be flexible. We also know that we all work best in our own way, which is why we offer the FlexSmart flexible work schedule. FlexSmart operates under a simple premise – associates are expected to consistently perform exceptional work that advances our mission and vision. Sometimes, this can be accomplished with a flexible work schedule. A flexible schedule can mean many things, including telecommuting and working non-traditional business hours. This policy only applies to PetSmart associates at the Phoenix Home Office and the Canada Home Office. Some teams may have supplemental guidelines, such as the Services Reservation Center and IT Solutions Center.

Shifting from Part-Time or Full-Time

  • Shifting from part-time to full-time may impact your eligibility for medical, dental, vision, and other benefits, including paid time off accruals. To learn more about what you’re eligible for visit the Benefits 101 page.

Rehired by PetSmart

Full-time associates returning to work at PetSmart:

  • Returning to work at PetSmart within 30 days of separation will bridge your service back to your latest hire/rehire date. Any unused balance for vacation, sick, and discretionary hours will be reinstated including the original eligibility date. The previous health benefits elections are reinstated on the first day of the month after your rehire date, and in accordance with the reinstatement rights under the Affordable Care Act, if you are rehired into a full-time position.
  • Returning to work at PetSmart more than 30 days after separation will result in eligibility for benefits after the same waiting period as any other new PetSmart associate and in accordance with the reinstatement rights for the Affordable Care Act.

Making Changes to your PetSmart Benefits

At the time of a qualified event, you may be eligible to change your enrollment, including adding or removing dependents enrolled in your benefits plans. Qualified events include: birth/adoption of a child, marriage, divorce, death, or gain/loss of other coverage. If you’re eligible to make a change to your benefits, you must make the change within 45 days of the date of the event, or you will need to wait until the next open enrollment. If you report the change within the 45-day period, the change in coverage takes effect on the date the Benefits team receives the completed documentation (except for the birth of a child, which is the date of birth). To request the change, submit a Qualified Status Change Form (QSCF) with the supporting documentation to the Benefits department within 45 days of the event.

Illness or Injury at Work

If you’re injured at work, immediately notify your leader on duty for assistance in seeking appropriate workers’ compensation medical attention. Workers’ compensation coverage is provided by PetSmart at no cost to you.

PetSmart is required to report all workers’ compensation claims to the applicable state authorities within a certain time period. To determine the severity of the disability, you may need to be examined by a physician. For questions about an existing claim, please call Sedgwick at 1-888-287-7387.

Separation from PetSmart

If you leave PetSmart, you are no longer eligible to participate in the SmartChoices Benefits Plan. Generally, coverage ends at midnight on the date your employment with PetSmart ends. COBRA Coverage

At the time of separation from PetSmart, you may be eligible for Consolidated Omnibus Budget Reconciliation Act of 1985 (COBRA) to extend the medical, dental, vision, and Health Care FSA coverage you are enrolled in on the date your employment ends. Under COBRA, you will pay the full cost of coverage plus a 2 percent administrative fee. For COBRA rates, read Benefits and Pay Information for Separating Associates.

You should receive COBRA information within 14 days after your employment ends. If you do not receive this information, please call WageWorks, Inc., a HealthEquity company, at 1-877-722-2667. For more information about COBRA, review the Initial Notice of Cobra Rights document.
