When you enroll in a PetSmart medical plan, you automatically receive prescription drug coverage through the CVS Caremark® prescription benefit program. You can use your CVS Caremark prescription benefit ID card at participating pharmacies or when ordering prescriptions through the CVS Caremark Mail Service Program. Go mobile with the CVS/Caremark App for access to your ID card, to view prescription history, and to track your mail order.

Through the pharmacy plan you have access to additional health programs that provide support and guidance, tools and resources to help manage your chronic or complex health conditions. You can learn more about all of the available programs on the Health Programs page.

Part-time associates that are eligible for medical coverage will be contacted by the PetSmart Benefits Team regarding your enrollment window. Take time to consider your needs for the coming year and use the tools and resources described below to help make the right choice for you and your family.

Mandatory Maintenance Choice

Mandatory Maintenance Choice offers you savings when it comes to filling long-term prescriptions. The plan will cover long-term maintenance medications only if you have 90-day supplies filled through CVS mail service or at CVS or Target pharmacy. Maintenance medications are drugs you take for regular daily use or for ongoing conditions such as diabetes, high cholesterol, allergies, high blood pressure, etc.

How do you move your current 30-day prescription to Maintenance Choice? Simply request your physician to provide a new prescription written for up to a 90-day supply with 3 refills to cover a 12-month period. Submit the new prescription to any CVS or Target retail pharmacy and receive your medication for the same low mail service copay. 

The amount you pay may vary depending on the type of drug you need and the method in which you fill your prescription. The prescription plan comparison chart below will illustrate some of those differences. Mail Service Prescription Drug Program

The caremark.com mail service program can save you time and money when you order up to a 90-day supply of prescription medications (taken for ongoing treatment of conditions such as diabetes, ulcers, arthritis, and heart problems.) Some medication may be limited to a 30-day supply. Have your doctor write a 90-day supply prescription and indicate the number of refills for your maintenance medication.

The best way is to call your doctor and ask to have a new 90-day prescription sent to CVS Caremark Mail Service Pharmacy electronically. If you’re having trouble reaching your doctor, sign into Caremark.com and select Start Rx Delivery by Mail to get started.

Note: You cannot access Start Rx Delivery by Mail from the CVS Caremark app. You may access this service from a web or mobile browser.

Some medications are covered by the no cost preventive services list, and with a prescription will be covered with a $0 copayment. When using a manufacture coupon or discount to purchase prescriptions, your deductible amount will be calculated based on your actual out-of-pocket expense.

*Preventive care is important for keeping you healthy today and tomorrow, and prescription medication that helps treat chronic conditions is necessary. When enrolled in the HDHP plan coverage for certain generic prescriptions classified as “preventive care prescriptions” are covered whether the deductible has been met or not. These specific generic medications will always bypass the deductible and you will be responsible for the applicable coinsurance. These include drugs that treat chronic conditions such as high blood pressure, high cholesterol, diabetes, etc. Check out which drugs are on the current list.

PrudentRx Copay Program

Members who are experiencing chronic or complex conditions may be eligible for the PrudentRX Copay Program. To learn more, contact Caremark Specialty Pharmacy at 1-800-237-2767.

PrudentRx Co-pay Program for Speciality Medications-PPO1, PPO2, HDHP, BIND, and Out of Network Plan

Prudent Rx provides a comprehensive and cost-effective prescription drug program for you and your covered family members with the PrudentRx Co-Pay Program for certain specialty medications. The PrudentRx Co-Pay Program assists members by helping them enroll in manufacturer co-pay assistance programs. If you enroll in the PrudentRx Co-Pay Program, your out-of-pocket cost for prescriptions covered under the PrudentRx Co-Pay Program will be $0. Otherwise, medications in the specialty tier will remain subject to a 30% coinsurance.
