Basic Life Insurance
PetSmart gives all eligible associates a basic level of life insurance equal to 1x annual base salary.* You pay nothing for this coverage; however, this is a taxable benefit that appears on your pay statement as “ER BN Payment,” which means “Employer Benefit Payment.”
The maximum amount of life insurance for you, including the company-paid basic level of life insurance and any additional coverage you elect and pay for, is $1 million.
Optional Life Insurance
Canada offers our associates Optional Life Insurance through Manulife Financial.
- Policy number: G0114822
- Benefit amount allowed: increments of $10,000 to a maximum of $200,000
- Termination age: your benefit amount terminates at age 70 or retirement, whichever is earlier
Option Life Insurance is the responsibility of the associate.

Dependent Life Insurance
Canada offers our associates Dependent Life Insurance through Manulife Financial.

Accidental Death & Dismemberment (AD&D) Insurance
Accidental Death & Dismemberment coverage gives you extra insurance protection in case of any certain injuries or death. AD&D is company paid and you are automatically enrolled.
