Dental Plan

The PetSmart dental plan pays for expenses up to the current year’s dental association fee guide limits. While you’re a member of the plan, you’ll be reimbursed for all eligible expenses, up to the annual per-person maximum. Be sure to check if your dentist follows the current year’s fee guide (and doesn’t charge more). Charges exceeding those amounts cannot be reimbursed.

Dental Coverage Highlights

Maximum Coverage

Basic services (Level I -preventive)

The plan pays up to 90% of the cost of treatment. These services include:

  • checkups
  • extractions
  • some oral appliances such as space maintainers
  • fillings
  • minor surgery
  • denture repair (rebase and reline)
  • 6 month recall for cleaning

$2,500 maximum per person each calendar year combined with Basic Level II, III, and IV

Basic Services (Level II – supplementary)

The plan pays up to 80% of the cost of treatment. These services include:

  • oral surgery (not included in Level 1)
  • periodontics (gum work)
  • endodontics (root canals)

$1,000 maximum per person each calendar year; combined with Basic Level I and II

Major Services (Levels III & IV)

The plan pays up to 50% of the cost of treatment.

These services include:

  • dentures
  • crowns
  • bridges

$1,000 maximum per person each calendar year; combined with Basic Level I and II 

Orthodontics (Level V) 

The plan pays up to 50% of the cost of treatment. Orthodontics include:

  • braces
  • retainers

$2,000 per person lifetime maximum
