When to Enrol

How to Enrol in Coverage
- To enrol in optional life and/or dependent coverage, complete and return the Group Benefits Application for Change form to HRSharedServices@petsmart.com.
- Note: If dependents are enroled more than 31 days after first eligible, benefit coverage within the first year may be reduced
- To enrol in the Group Retirement plans (RRSP/DPSP), follow these steps.
- Naming of beneficiaries must be completed for Life/AD&D insurance and Group Retirement.
- Life and AD&D: You can designate your beneficiaries by submitting the Group Benefits Application for Change form (linked above), or online in “My Account” via the Manulife portal.
- RRSP/DPSP: You will be asked to designate your beneficiaries during the enrolment process.
How to Waive Coverage
- To waive Extended Healthcare and/or Dental coverage for yourself, complete and return the Group Benefits Application for Change form (linked above) to HRSharedServices@petsmart.com.
- Long-term disability is required and may not be waived
- To waive Group Retirement participation, complete and return this waiver form to HRSharedServices@petsmart.com.
