PetSmart offers medical insurance to our eligible Hawaii associates through HMSA participating providers.

The health care coverage is a Preferred Provider Organization. This means you have medical benefits for your health care needs including office visits, inpatient facility services, outpatient facility services, and other provider services. This coverage offers you flexibility in the way you get medical benefits.

Member coverage premiums vary by coverage level. In the chart below, we’ve provided the medical plan weekly premium costs by coverage level.

When to Enroll


What to expect

Within 45 days of your hire date or the date your status changed from part-time to full-time.

  • Review your SmartChoices benefits options and decide which plans and coverage are right for you and your budget.
  • Enroll for benefits. Enroll online through HR Connect.

    Be sure to print a copy of your enrollment confirmation or include your personal email and receive your confirmation by email. You will not receive a confirmation statement in the mail.

First day of the month following your hire date

  • Coverage begins for salaried associates.
  • Coverage begins for full-time hourly associates.

Every fall

  • Open enrollment is your once-a-year opportunity to review and make changes to your benefits choices.
  • Your new benefits elections become effective the following Jan. 1.
