Dependent Audit: Action Required by March 24, 2009
If you cover dependents under PetSmart’s medical, dental or vision plan, you will be asked to provide documentation verifying your dependents are eligible for coverage.
In late February, you will receive a letter outlining the audit process and the documentation needed at your home address. If you don’t provide the required documentation by March 24, 2009, coverage for your ineligible dependents will be canceled. Learn more.

In 2009, all associates covering dependents will be asked to provide documentation to verify eligibility. For example, if you cover your husband or wife, you may be required to fax a copy of your marriage license. If you don’t provide the required documentation within the specified time period, coverage for your ineligible dependents will be canceled. Look for more information in early 2009.
